PARENT/GUARDIAN – PROVIDER MUTUAL AGREEMENT
A nonrefundable deposit of $60 will hold the student’s space in the program.
Once the contracted week begins, a paid fee is nonrefundable, whether the student attends.
Payment will be paid in the form of: Credit/Debit Card, Online Payment, Cash, Check, DSS Voucher.
Student activities will be provided Monday-Friday 7:00 a.m. – 9:45 p.m.
Tardy Pick Ups- Rate of $5.00 every 15 minutes is charged for a student picked up after Closing Time.
Community Learning Academy follows the Central Islip School District School Calendar, and therefore the Academy will be closed on holidays and/or school closings.
Parents must submit a copy of the student’s current School Medical Documents.
Termination Procedure: The Academy or parent/guardian can terminate this agreement with at least a week’s written notice.
By checking this box, You Understand that photographs/videos taken during the program session may be used for publication, and for the future website.
I give permission for my child to be photographed during the program.
