Terms and Conditons  

  1. You must register your child for the entire weekly series. See form below. 

  2. Provider follows Central Islip School District Calendar, and therefore the Academy will be closed on school holidays and/or school closings. 

  3. Termination Procedure: The Academy or parent/guardian can terminate this agreement with at least one week’s written notice.

PARENT/GUARDIAN – PROVIDER MUTUAL AGREEMENT

A nonrefundable deposit of $60 will hold the student’s space in the program. Future payment is due BEFORE the start of each exploratory week. Trip reservations and busing require confirmations

This agreement pertains to your child’s enrollment in Exploratory Activities beginning (check box for appropriate weeks}:

Total Weeks and Payment Amount

  1. Once the contracted week begins, a paid fee is nonrefundable, whether the student attends.

  2. Payment will be paid in the form of: Credit/Debit Card, Online Payment, Cash, Check, DSS Voucher.

  3. Student activities will be provided Monday-Friday 9:00 a.m. – 4:00 p.m.


  4. Tardy Pick Ups- Rate of $5.00 every 15 minutes is charged for a student picked up after Closing Time.


  5. Community Learning Academy follows the Central Islip School District School Calendar, and therefore the Academy will be closed on holidays and/or school closings.


  6. Parents must submit a copy of the student’s current School Medical Documents.

  7. Termination Procedure: The Academy or parent/guardian can terminate this agreement with at least a week’s written notice.
